Evening of Entertainment Talent Application

By applying for the 2025 Evening of Entertainment:
PHOTOS; I/we give Access-4-All, llc and the Recreation Council permission to take and use my/our photograph(s) as deemed appropriate to promote this and future Evening of Entertainments, for an infinite period.
To the fullest extent permitted by law, I/we indemnify and hold harmless Access-4-All, llc and the Recreation Council of Greater St. Louis, and their directors, officers, consultants, agents, employees and volunteers from and against all claims, damages, losses and expenses, including but not limited to attorney’s fees and court costs, arising out of or resulting from my participation in this event, including damage, loss or expense is attributable to bodily injury, sickness, disease or death, or personal injury, or to injury to or destruction of tangible property or to others involved in the event.
I/we certify that at least one performer has a disability.
